Proxmox Upgrade on Zpool 1+1+1+0 - Fails

I have a Proxmox system where I built:
1 Mirrored Vdev
1 Mirrored Vdev
1 Mirrored Vdev
1 Mirrored Vdev
With the OS installed across all of them, After an apt-get dist-upgrade, the system boots to a grub recovery screen.

Any clue what is wrong with it?

what does the error message say? what does "ls" and "set" output?
