I dont think this problem is related to your controller. We run around 20 Proliant servers with smart array controllers and we dont have any issue with proxmox.
Yes, if a newer firmware is available, perform the update.

Have you tried with other cables? Maybe you have bad cables.

I believe the P400 only supports 1.5 Gbps in SATA mode, 3.0 Gbps in SAS mode.

When batteries are fully charged your cache should be enabled.

Btw. your cache ratio seems weird. You have 0% Read / 100% Write I would expect something like 50% < Read < 75%

Try /var/lib/vz instead. /dev/mapper/pve-data is the block device while /var/lib/vz is the file system.
