Intel DC P4510 PCIe Passthrough Not Working

Hi all,

I am struggling to get 1 out of my 4 Intel U.2 disks to passthrough to any VM. The idea is to get all four disks passed through to a TrueNAS VM, which I previously had working on this same machine with a PLX switch card. I destroyed the ZFS pool in TrueNAS because I no longer needed it, but now I am trying to get it back up and running.

3 of the 4 P4510 disks will pass through will no issues, but the fourth will not. I have tried swapping cables around, swapping ports on the PLX card, and swapping the PLX card for another one. It's always the same S/N disk that won't pass through, but it is recognized and visible in Proxmox. I can even create a ZFS pool in Proxmox with all four disks, and put data on them. It is solely the passthrough that is not working. I have wiped the disks in Proxmox several times as well, but the one disk is never recognized inside of any VM.

Any suggestions would be much appreciated.

Hardware:
Motherboard - Supermicro H13SAE
CPU - 7950x3d
RAM - 64GB 5600MHz unbuffered ECC
GPU - Various depending on testing - 9060xt or Quadro P6000 usually
Disks - Proxmox OS is on a 250GB Micron Sata SSD; VM OSs are on Micron 1TB; VM data is split across several other SATA drives (all LVM); 4x Intel DC P4510 for passthrough
PLX Card - Both I've tried are some Chinese non-branded models that have 8 PCIe gen3 ports with 4 lanes per port on the disk side, and x16 on the motherboard side (ran at x8 because I have a GPU installed)
