Background to the problem:
I've been running PVE on a Dell R220 machine for about three years, and upgraded from PVE 4.3 to 4.4 without any issues.
Recently however, when rebooting the host to perform PVE kernel updates, the machine boots to a flashing cursor and hangs there. Last time, it went in to loop: getting as far as the end of the POST routine I think then starting to boot again. The only way I could get it to to boot correctly was to go to the BIOS boot menu and explicitly tell it to boot from the hard drive.
The problem occurs both over the serial console as well as when I'm at the machine with a physical one.
I suspect some issue with fstab or grub perhaps but I've not changed anything.
