As you have problem with the hard disk:

1. what kind of harddisk/storage controller do you have?
2. which filesystem do you choose for your intallation (ext3/4, xfs or zfs)?

1. HARD DISK -> Sata /64MB cache WD RED 1.0TB
2. I downloaded the ISO in the website: http://www.proxmox.com/en/downloads/item/proxmox-ve-4-3-iso-installer

- which filesystem do you choose for your intallation (ext3/4, xfs or zfs)?

please clean/wipe your harddisk completely (e.g. with a linux live cd) and and try the ISO install again.

Depends on your needs. If you have the right hardware (Fast SSDs and a lot of RAM), I prefer zfs.
