Unable to start VM after FreeNas Upgrade

I recently upgraded from FreeNas-11.3-U5 to TrueNAS-12.0-U1. After running this update I am unable to run and VM's that were using iscsi and LVM storage from TrueNas. Why I attempt to start VM 101 I receive the following error:

Metadata location on /dev/sdc at 323584 begins with invalid VG name.
Failed to read metadata summary from /dev/sdc
Failed to scan VG from /dev/sdc
Volume group "Jarhead-VM-LVM" not found
can't activate LV '/dev/Jarhead-VM-LVM/vm-101-disk-0': Cannot process volume group Jarhead-VM-LVM

I tried another VM, 102, and receive the following error:

Failed to scan VG from /dev/sdc
Metadata location on /dev/sdd at 144896 begins with invalid VG name.
Failed to read metadata summary from /dev/sdd

Failed to read metadata summary from /dev/sdc
Failed to scan VG from /dev/sdc
Metadata location on /dev/sdd at 144896 begins with invalid VG name.
Failed to read metadata summary from /dev/sdd
Failed to scan VG from /dev/sdd

Incorrect checksum in metadata area header on /dev/sdc at 4096

Any thoughts on what could be causing the issues?

Is my assumption correct that this is connected through iscsi?
If yes - are the connection established?
Maybe your upgrade has modified IQN so the connection fails.
 
Yes, the connections are through iscsi. The connections appear to be established as Proxmox is showing the storage as available.
 
Have you tried to confirm accessibility by writing something to the storage?
