I have been using my system without issues for at least 4 months now and all of the sudden last night it flickered to a white screen and then came back before the monitors went black. The GPU on passthrough is an EVGA 3080ti that will detect inside the windows VM but goes between an error 43 or an error 12. I have tried Updating Windows as well as the GPU drivers with no success, I tried removing the card from the VM and re-adding it as well as removing it from the system and re-installing it all had no effect. Today I created a brand new Windows VM and installed just Windows 10 and the Nvidia Drivers and still getting a consistent error 43. I have tried both the Game drivers and the Studio Drivers. I am not sure if I should send the GPU for RMA or is it a software issue with Proxmox and or the windows Install(s).

Maybe the Machine Version changed, if it is not set to a fixed version under the Hardware settings of the VM? You could try booting with an earlier version.
There have been some pve-kernel updates lately that broke passthrough temporarily. There is also pve-kernel-5.15 that brings simplefb, which can break GPU passthrough if the same GPU shows BIOS and boot messages and the host console.

The Machine Version was the same, I ended up getting it to boot again after removing the card and installing it in another slot on the board. Still not sure what caused the error but I plan to rebuild the whole system and move from Seabios to UEFI as my other system that is using UEFI has not had any issues and it is much older and more cobbled together machine. I haven't tried the latest opt-in kernel at all though I am using the non-subscription repo.
