Proxmox Won't boot. Buffer I/O error drops to (initiramfs) busybox shell

Hi !

I was running proxmox on a Opeiplex mini for about 2 years,all of a sudden VMs stopped working and I could not reach Proxmox server anymore.
Rebooted Promoxmox server and it drops to a busybox shell prompt (initramfs).
The error on the screen is:

Buffer I/O error on dev dm-1, logical block 0, async page read
Buffer I/O error on dev dm-2, logical block 15532016, async page read

When I run fdisk the same error shows on the screen
Mount shows:

rootfs on /
sysfs on /sys
proc on /proc
udev on /dev
devpts on /dev/pts
tempfs on /run

Fstab is empty

Searched the forum but could not find any solution.
