This could be the "old" ZFS.

so i boot in " rescue mode " with the Old 12GB Hard Drive connected including the 4 x ZFS Raid Drives ?
Yes, then try to import the old pool via:

zpool import -R /rpool rpool

Does that work?
 
So, your system is working from the old SSD and you could start everything?

Next would be to add a new 120G disk and follow the link in comment#2

When booting from USB stick or the old 120GB SSD? If your old system SSD is now booting again, remove the USB stick, put in the new 120GB SSD and try to boot from the old 120GB SSD.
Then you can wipe the new 120GB SSD and follow the wiki on how to add the new SSD replacing the failed EVO so you get a working mirror again.

Did you plug in the new disk and reboot? Why is there a zpool called rpool on the disk? I thought it was installed with LVM?

First, we should go back to the state in which the rpool was mounted and you said that it booted. Then we first copy off the /etc/pve files, they should be mounted and we have the config and the data on the pool so that we can try to fix the machine. If the files are not mounted, please copy off the file /var/lib/pve-cluster/config.db.
 
So whats your lsblk and ls -la /dev/disk/by-id/ output when booting from that 120GB SSD that is now booting?
