in plex i can see this error :

[Transcode] TranscodeUniversalRequest: unable to get container: /livetv/sessions/c5943103-bedc-4a49-9e1e-5affc5e0a801?includeBandwidths=1&offset=-1&X-Plex-Incomplete-Segments=1&X-Plex-Session-Identifier=vyuq7tf1avbt3gee40yxmmtx

but when i look my video on it the processor is full
my guess is simply that your processor cannot keep up with transcoding (or what else plex does)
i see that you use the default cpu settings, you maybe want to set your model to one with more cpu capabilties (e.g. host) this may decrease the load
also you maybe want to think about passing through a gpu, afaik plex can make use of a gpu for transcoding
