Need Help Read-error on swap device. Kernel panic

Hi all;
need help for my proxmox v8.24 got "read-error on swap device and then with kernel panic - not syncing". And my server got error . After hard reboot going nomal again but in 10 - 12 hours than get that error again and need to reboot to make normal.

Any advice to fix this ? Thanks

Disk is going bad.

Replace disk, reinstall Proxmox, restore VMs / LXCs from backup
