As said looks like indicates an issue with one of your disks. In this case, you have to check the disks using smartctl tool

Hello, sorry for the late reply, I have 8 disks 6 with raid 5 and device ID of the 6 disks are (0,1,2,3,6,7) and other two disk are not usabale because they need to be replaced when I create proxmox I used the valid 6 disks (one virtual disk).

I have run the smartctl tool to check the health of my 6 disks and here is the output (please note the output for 0,1,2,6,7 are same), I think my disks are fine, Please advise if is there any other solution for the bootable issue.
Please note that in device id=3 the SMART Self-test log wasn't completed successfully, I don't know if it causes a problem or not.

Disk #3 could not even finish the short SMART-test.

Would suggest to run a long SMART-test [1] on all of your drives.
The raid-controller should also have some sort of disk-/raid-/volume-/parity-verification, I assume. Would suggest to run this too; maybe even before the SMART-test(s), but not both at the same time!

I hope you have backups...

Edit: And of course, prepare to replace those already faulty drives.
