I/O error

Hey. At first, sorry for my poor English skills. My home server has proxmox installed. Initially, everything worked perfectly for a few months but now there are error messages as shown in the attached image. When proxmox is restarted everything will work normally. After a few hours, the web gui and ssh connection will not work and an error message will appear as shown in the attached image. All running virtual machines and containers are operating normally. The only way to access proxmox with a browser or ssh connection is shut down and restart proxmox. Is there a fault on the hard drive or what could cause such a problem?error.jpg
 
This does sound like a hardware problem.
How is the server set up? Hardware RAID? Single disk? Software RAID (ZFS)?

What are the SMART values of /dev/sda?

As your screen show you have a disk/controller problem as @aaron already said. As I can see, are only 2 different sectors with problems. I would suggest for your safty to do like this:

1. make backups for all yours VM and/or CT, and also at least for your /etc from PMX host
3. run as @aaron said:

smartctl -a /dev/sda

2. Boot from a clonezilla live cd and run for several times:

smartctl -t long /dev/sda
