I got PMG working but also not. What am I doing wrong?

So background.

I've set up a PMG gateway for front end for an Exchange server. Unlike the default config, we have the PMG server sat outside of our network/firewall on a cloud server as our ISP cannot seem to provide an IP that isn't blacklisted.

Set up the PMG to relay incoming mail to the Exchange and outgoing mail from the Exchange via smart host. Mail delivery to and from is flawless. My problem is spam filtering. For some reason PMG is classifying *all* mail as outgoing mail. This is less than idea from a spam filtering perspective. Any idea what i've configured wrong?
