I would like my blacklist items to be deleted. For some reason it just puts it in the spam quarantine. I thought I could add it to the filter rules but there isnt a delete under the action items. Is this just not possible?

please share the logs of such a mail, and your rule-system where the item is in the blocklist - then we can see why it's not working as you'd expect..

if a user blacklists an email address aaa@exemple.com, the next mails from aaa@exemple.com will go in the quarantine with a score of 100.
If we send a daily report for this user, he may receive a report just because of mails coming from aaa@exemple.com, but he does not want to see ithis anymore.
So the question is: is it possible to configure to trash these mails?

the user block/welcome lists only tweak the spamscore - what happens with those mails depends on the rules configured by the admin.
If you configure a rule to block mails with a spamscore of e.g. 50 (with higher priority than the one that puts them in quarantine)- the mails will be blocked and not quarantined.
