Create OSD Crash and Reboot

I have replaced an SSD and trying to add the new disk as an OSD - but everytime i do the host crashes and reboots.

I have upgraded to the latest version and tried doing from the command line and does the same thing.

Why is this happening?
 
I am getting the following errors and rebooting also.

the vendor_id: 0x8086 seems to be Intel
device_id: 0000:00:00.0 seems to be the host bridge/root complex

So basically, you have an hardware error, that is most likely located at the northbridge. It could be a CPU, memory or motherboard issue, contact the hardware vendor for support. Also worth a shot, is microcode and BIOS (motherboard & IPMI) updates.
