Console Error VE 6.0

Just fired up a new box I got and put 6.0 on it.

I see the attached screen shot at console.

Any ideas what this looks like to be?

Something send illegal requests to the drive and it reported back. You also have I/O errors on some blocks.

Does this happen before? (you can check via dmesg or in /var/log/syslog).

This relatively a new server as well new ssd.
Do you think the I/o error is a sign that this server hardware is no good?
