Just grab this ISO and make a 2nd disk to access it during Win installation.

Yes, it will stay on the recovery node. Also, you don't just reboot the protected node to test it. It literally must be unplugged. Also, make sure your fencing is working well in the Datacenter->HA tab.
I have used the software version since it was added by the devs and have had no problems at all with it. 5 node setup was the largest cluster, though. Currently using version 4.2 and no issues.
A simpler way is just to add the serial=1234 or whatever to the .conf file for your VM. Make sure you power cycle (not a simple reboot) the VM to reset the value.

What is the operating system? If it is a Windows based system, you could boot off of a Windows setup disk and attempt a repair of any Windows installation it finds on the disk.
