I have this in the log:
Dec 20 14:16:07 pve pvestatd[1438]: command '/usr/bin/iscsiadm --mode node --targetname iqn.2000-01.com.synology:cnet-nas-dd2.Target-3.8dfdd49567 --login' failed: exit code 15

I disabled the IPv6 on the Synology iSCSI interfaces.

PVE still sees the IPv6 paths.
10.111.111.10:3260,7 iqn.2000-01.com.synology:cnet-nas-dd2.Target-3.8dfdd49567
10.111.111.253:3260,8 iqn.2000-01.com.synology:cnet-nas-dd2.Target-3.8dfdd49567
[fe80::9ab7:85ff:fe00:dd50]:3260,8 iqn.2000-01.com.synology:cnet-nas-dd2.Target-3.8dfdd49567
[fe80::9ab7:85ff:fe00:dee4]:3260,7 iqn.2000-01.com.synology:cnet-nas-dd2.Target-3.8dfdd49567

Curious if I can fix this.

PVE still sees the IPv6 paths.
PVE uses standard Linux userland tools (iscsiadm) for Discovery. You can manually run the tool (add -d debug_level for more details) and see what Synology sends back.

If you think that you disabled the IPv6 but your NAS still provides them in the answer, I would reach out to your NAS support channel.
