Who do I contact to get the subscription key reissued ?

I saw in one of the posts that if you reinstall Proxmox the key does not work anymore (server ID error), this is exactly what happened to us. We used the same hardware everything except we had to reinstall.

Just contact proxmox support - they will re-issue the key for you.
