VM's going into a state of io-error

Hey all, I want to get a few opinions on the issue I am seeing. Any new VM's which I try to setup are going into a state of "io-error". Doing a bit of reading and it seems this almost always indicates a hardware issue. Has anyone else hit an issue like this? According to megacli, everything is optimal.

UPDATE: Seems this only happens with VM's which I am trying to migrate from .vmdk. In the past this has worked, but now I am getting the io-error. I am using the vmware-vdiskmanager to convert the vmdk to a single grow able file, then I am dd'ing it to the raw image file created with the VM.
