Mail Subject wont get modified with SPAM

Hi all,
when a Mail get classified with SA score >=3 the Mail subject wont get modified.
I didn't find any bad configuration options.
May someone can give me a hint?

Could you share the logs of such a mail, that hat a score > 3 and that did not get it's subject modified?
