I had the same issue after adding a few OSDs and was able to fix it by enabling the ceph balancer:
ceph mgr module enable balancer
ceph balancer on
ceph balancer mode upmap
After doing this, my pool went from 93% of used space to 58%, so it definitely helped a lot.
I was able to fix it by setting "KVM Hardware Virtualization" to "No". Windows installation went through just fine afterwards. After first boot, I set it to "Yes" and installed all missing drivers in Device Manager from VirtIO ISO.

I tried to install Windows Server 2016 in a Proxmox 4.4 VM, but I keep getting a bluescreen without any error code or additional information at first boot. Windows Server 2012 installation is working just fine.
