    I assume your server has Smart Array B320i/512MB FBWC controller inside (onboard). If this controller is not supported by Proxmox, I would spend some bucks for a good controller. Software Raid might work for test or development environments but not productive use.

    Have you tried to set the I/O scheduler of the KVM guest to NOOP ? Just in case you run a kvm.
