I assume your server has Smart Array B320i/512MB FBWC controller inside (onboard). If this controller is not supported by Proxmox, I would spend some bucks for a good controller.
Software Raid might work for test or development environments but not productive use.

I might be wrong, but your disks HP DG146ABAB4 spining on 10k and has just a transfer Rate of 3.0 GB/sec. The HP Smart Array P410 is capable for SAS disks with a transfer Rate of 6.0 GB/sec. Using a faster Disk spining 15k might improve the performance as well.

This is an outdated Repo for the 3ware controller CLI and 3dm2 webtool. Remove the lines in /etc/apt/sources.list.
This repo is not longer needed. Go to the 3ware support page and download the latest cli/3dm2 installation package. This installs pretty easy with a shell script on all linux...
