got the same problem with proxmox ve 4.3 - I got it working by reformatting the whole USB drive (, not a partition! the drive seemed to have a GPT instead of a MBR!) with the Ubuntu drive manager, using a FAT/MBR. Then I copied the iso onto the stick with dd again, and booting worked.

Got to this thread to google, sorry to resurrect. in Ubuntu 16.04 usb-imagewriter is not available any more. And the dd copying produces a broken USB image. (Yes I triple checked the md5sum).
