EXT3-fs I/O error on dm-4

Hi guys!

I'm getting some I/O errors in my syslog on device "dm-4" and I'm not sure if it's symptomatic of a bad disk or if I've just got a stale backup or what. After reading through some related forum posts here are some outputs that might be helpful (I highlighted anything related to dm-4 in red bold):


Determining what dm-4 is:
